EMIS Web is our clinical system, which includes things like patient records and the appointment book. It is cloud-based, and records can be accessed from anywhere with the necessary computer setup. We can also access patient’s community & secondary care records.
FYI - as of March 2025, EMIS is rebranding as Optum. Our clinical system is staying as EMIS Web for now, but you may notice Optum branding in some places.
Getting started
The first time you log-in to EMIS Web you must change your password (i.e. the password IT send in your welcome email is only used for your first log-in. After that, you must set your own password).
You must also follow the instructions in the toggles to verify your email, add your security details, and sync your Smartcard.
If you have multiple EMIS log-ins for different sites, you must follow these steps for every site.
Opening EMIS
With smartcard:
- Before opening EMIS, insert your Smartcard
- Enter your passcode, and select the site you wish to access
Without smartcard:
- If the ‘Log in with Smartcard’ window appears, Click ‘Cancel’
- Another pop-up will prompt you to enter your username and password for EMIS
Changing organisation/CBD
Each of our surgeries has a unique organisation ID / CBD (you can find them all on Our Sites). You can log in to a different surgery’s EMIS Web by changing the ‘Organisation ID’ when signing in.
- If the EMIS Web log in window is open:
- Click ‘Change organisation’
- A new window will open → enter the organisation ID/CBD that you want to open and click ‘Restart’
- If the Configuration Switcher opens, select the organisation ID/CBD that you want to open from the dropdown and click ‘Go’
The EMIS Web Homepage
The EMIS Web homepage is the first page displayed when you access EMIS Web. The homepage has three main sections (circled in red in the image below):
- Quick Launch Menu → shortcuts to frequently used sections on EMIS (which can be edited by clicking the settings cog icon ⚙️)
- Organisation Notepad → updates from others within your surgery
- Latest updates from EMIS Now → any updates from EMIS or in some sites their PCN
At the top of the screen there is the Quick Access Toolbar, containing shortcuts to frequently used sections of EMIS
Raising a panic alarm
The panic button 🛑 is in the screen’s top-right corner. If you need to notify all other staff in the practice that you need help - raise an alarm by double clicking this button.
How to customise the Quick Access Toolbar
- Click the arrow next to the Quick Access Toolbar and click ‘Customise Quick Access Toolbar’.
- In the left-hand side of the pop-up, click on the icon that you want to add to your toolbar and click ‘Add’.
- This will move it over to the right-hand side of the pop-up.
- Click ‘OK’ once you’ve added any icons you want.
How to open a patient’s notes
You can search for a patient using any of their details that are recorded on EMIS, e.g.:
- Name
- Date of birth
- NHS number
- Address
- Telephone number
Double click the patient that you want to open.
How to open Consultations
click ‘Consultations’ in the the Quick Launch Menu on the homepage, OR
click the ‘Consultations’ icon in the Quick Access Toolbar
The main part of ‘Consultations’ that you’ll need is the right-hand side. This pane displays all consultations that have been added to the patient's care record, including:
- Consultation date, location and clinician
- Presenting problems
- Examinations
- Any additional details
- Any documents attached to their record
- Referrals
- Medications
- Results
How to add a new consultation
- A pop-up will appear to confirm the date, time, consulter and location. You can check the box next to ‘Store as default for this session’ to stop the pop-up appearing for the rest of your session.
- Click the relevant tab (e.g. Problem, History, Examination) and enter the reason for adding the consultation using relevant codes.
- Click ‘Save’ in the toolbar to save and close the consultation.

Hiding a consultation
- Open the consultation that you want to restrict
- Click ‘Online Visibility’, and select ‘Do not display on the patient’s online care record’
- Complete and save the consultation
How to open the Appointment Book
click ‘Appointment Book’ in the the Quick Launch Menu on the homepage, OR
click the ‘Appointment Book’ icon in the Quick Access Toolbar
- On the calendar click on the date you want to access, or click ‘Today’ to go to today’s date
- Click on the ‘All Clinicians’ button under the calendar to see all available clinicians
Adding & removing clinician from view
If you don’t have the ‘All Clinicians’ button, or if it’s not showing all available clinicians:
- Click the ‘Appts Config’ button in the toolbar
- Click ‘Your Session Holder Filters’
- Click the green arrow to be able to view all available clinicians, then click ‘OK’
- You should be able to select ‘All Clinicians’ to view all clinicians at your site now!
Screen messaging
Screen messaging is a direct messaging service that can be sent to other users within your surgery.
How to send a screen message
- On the Quick Access Toolbar, click the screen message icon ✏️
- Enter your Subject and Message
- If the message is urgent, tick the Urgent box → this triggers an alert for the recipient
- Select the user(s) you want to send the screen message to by ticking the box next to their name
- You can show/hide offline users by ticking the ‘Show offline users’ box
- You can also search for a user using the ‘Find user’ search box
- Click ‘Send’!
Receiving a screen message
You’ll see a notification in the EMIS toolbar if you’ve received a screen message.
If the screen message is urgent, a pop-up will appear on your screen asking if you want to view the message now (View), or open it later (Close).
If you click ‘Close’ you can view the message later by clicking on the yellow notification bar.
To respond to any messages, open the message and click ‘Reply’.
- If multiple users need to receive a response, click ‘Reply All’.
If you’ve finished with a message you can click ‘Discard’ to delete it, but please note that discarded messages can’t be retrieved!

Patient warnings
These can be used for many reasons, for example:
- Noting a patient doesn’t wish to be seen by a specific GP
- Reminders to book translators or interpreters
- Specific access needs
- Consent given to discuss a patient with a family member or relative
How to add a warning:
- Open the patient’s record, and click on the top blue bar
- In the warnings section, on the right, click Manage
- At the top of the screen, click Add
- Type in the warning message
- Select when a warning should appear (Do this selectively, to ensure people find them useful)
- Click Ok, and then Close
The warnings can be added or removed by clicking the appropriate button in the Manage Warnings section.
Changing patient details
- Click ‘EMIS’ and click Registrations
- Select the required patient
- On the ribbon, click Edit Patient
- Edit the details as required click OK
You should never ask for proof of address (see also Adult Safeguarding Safe surgeries section and Gender Reassignment on Patient Records).
