What is this service?
The Lewisham Memory Service, provided by Lewisham and Greenwich NHS Trust, is a memory assessment and diagnosis service. The service aims to diagnose dementia early on, and help patients (and their carers) understand the condition, learn of treatment options and connect with appropriate support resources.
The Memory Service team can support patients by coordinating individualised care planning, prescribing medication (if appropriate), and making onward referrals.
Criteria
- 18 years old and over
- Memory problems likely to indicate dementia

Rejections, DNAs & other Quirks
- Investigations required, must be completed within 3 months of referral date β FBC, random glucose, TFTs, LFT, B12 and folate, ESR, CRP, U&Es, CA2+, lipids (triglycerides, HDL, LDL, total cholesterol), MSU
- This service is not suitable for patients with an existing diagnosis of dementia, unless there is a specific issue around medication tolerance
- The service canβt support patients with serious behavioural and psychological symptoms of dementia, but can refer on to other services for support
