What is this service?
The Southwark and Lambeth Memory Service (SLMS) is a memory assessment and diagnosis service. The SLMS team can support patients by coordinating individualised care planning, prescribing medication (if appropriate), providing post-diagnostic support and individual/group therapy.
Criteria
- 18 years old and over
- Mild to moderate memory problems likely to indicate dementia

- This service is not suitable for patients with an existing diagnosis of dementia
