What is this service?
The Dementia Clinical Nurse Specialist (CNS) service supports patients with dementia (and their carers) to improve their quality of life. Support offered includes:
- Holistic review of patient/carers needs
- Advice on management of physical, psychological and social issues
- Advice on crisis management
- Care planning
- Onward referral to other teams, such as community mental health teams (CMHT), where necessary
Criteria
- 18 years old and over
- Living at home (not in residential or nursing care homes)
- Registered with a GP in Lewisham or Southwark
- Formally diagnosed with a type of dementia

Rejections, DNAs & other Quirks
- Appointments are likely to be held at home, unless this does not suit the patient
- Patients will be rejected if they are (or have been in the last 6 months) Care Coordinated under a secondary mental health service or CMHT
- The service can’t initiate medications, as their nurses aren’t prescribers
