What is this service?
The Community Mental Health Team (CMHT) for Older Adults provide assessment, treatment and support for patients with dementia and/or mental health problems. The team includes psychiatrists, mental health nurses and occupational therapists who specialise in older adult mental health.
Criteria
- Lives in Lewisham
- Aged over 65 with mental health problems, or
- aged over 18 with a diagnosis of dementia

Rejections, DNAs & other Quirks
- Investigations required, must be completed within 3 months of referral date → MSU, FBC, U&Es, LFTs, TFTs, B12, folate, syphilis
- This service is only suitable for patients that live independently. For patients living in a care home facility, contact The Care Home Intervention Team via email at LewishamCHIT@slam.nhs.uk, or call 0203 228 8672
