What is this service?
The Healthier You programme is a free service which provides support to patients at risk of developing type 2 diabetes.
Over 9 months, the programme gives patients the opportunity to learn about lifestyle changes (healthy eating, losing weight, exercising) proven to help prevent diabetes. The programme can be completed in person, alongside others going through the same journey, digitally via the Oviva app, or remotely via Microsoft Teams.
Criteria
Inclusion
- Aged 18 years or over
- Registered with a GP Practice in South East London
- HbA1c 42-47.9mmol/mol (6.0%-6.4%) or fasting blood glucose (FBG) 5.5-6.9mmol/L within the last 12 months
Exclusion
- Is pregnant
- Has Type 2 Diabetes
- Has a diagnosed active eating disorder
- Had bariatric surgery in the last 2 years (even if the surgery has been reversed)
- Has moderate or severe frailty
How to refer
π€ Patients can self-refer via the online form
Patients must have their NHS number, blood test result (either HbA1c or FPG reading), and the date that the blood test was done (must be within the past 12 months)
Rejections, DNAs & other Quirks
- If a patient has a history of gestational diabetes, they are eligible for the programme if their HbA1c < 42mmol/mol or FPG < 5.5mmol/L
- If you are referring a patient over 80 years old, you must confirm that you consider the benefits of the programme to outweigh the potential risks
- The programme may not be suitable for patients that are unable to do light-moderate physical activity
- The remote programme is suitable for people who need BSL, have a visual impairment, need language support, or are women with previous gestational diabetes
- Online courses are available in the following languages: English, Urdu, Hindi, Arabic, Gujarati, Bengali, Tamil, Polish, Chinese, Punjabi, and Welsh
