What is this service?
This is a tier 3, intensive 1-year weight management programme for people who are living with complex obesity. Group sessions take place every 2 weeks throughout the year.
The programme includes nutrition education and advice about:
- changing your eating behaviour
- mindful eating, helping you to be more aware of how you feel when you eat
- goal setting
- psychology
- exercise
Choice of 2 group programmes:
- Balance - aims for slow, steady weight loss over one year
- Fast - aims for greater weight loss within the first 3 months under medical supervision, then introduction of healthy eating with follow-ups over next 9 months
Criteria
- Patients must be registered with a GP in Lambeth, Lewisham or Southwark
- Aged 18 or over
- BMI above 30 (if patient is white) or 27.5 (all other ethnicities), and diagnosed with type 2 diabetes within the last 2 years
- BMI above 35 (white) or 32.5 (all other ethnicities), if patient has 2 or more co-morbidities
- BMI above 40 (white) or 37.5 (all other ethnicities), with no co-morbidities

Patient must have access to the internet via laptop or iPad with video and microphone facility.
Exclusions:
- Pregnant or planning pregnancy
- Previously failed to complete a tier 3 weight management programme
- Having cancer treatment, palliative or end-of-life care
- MI or stroke within last three months

Rejections, DNAs & other Quirks
- Referrals must include the following, or they will be rejected:
- Blood results (within the last 6 months) → full blood count, lipids, thyroid function test, renal function test, liver function test, HbA1c
- Blood pressure
- BMI
- Height
