Objectives:
- Provide safe and effective care for all patients within the practice, and apply nursing skills to deliver high standards of care, adhering to evidence-based practices and national / South East London protocols.Β
- Implement evidence-based practices for both acute and chronic conditions, supporting management and preventative healthcare and assisting in managing clinical caseloads.
- Collaborate with the general practice team to address patient needs, improve healthcare delivery, and ensuring the highest level of patient care.
- Assist senior practice nurses in managing nursing resources and supporting practice goals, focusing on supporting the development of nursing services within the practice.

Administer routine childhood immunisations under Patient Specific Direction (PSD); support and guide parents.
Administer pneumococcal, shingles, and flu vaccines under Patient Specific Direction (PSD); educate patients on the benefits and potential side effects of vaccines.
Perform venipuncture to collect blood samples, ensuring accuracy in labelling and processing.
Administer B12 injections under PSD, ensuring proper documentation and patient safety.
Administer Zoladex, Prostap, and depot neuroleptic injections under PSD; monitor patients for side effects.
Assess and apply wound dressings; monitor healing and update care plans in collaboration with the practice nurse (simple cases only to be dealt with by HCA).
Apply and manage compression bandaging under the care plan provided by the practice nurse (simple cases only to be dealt with by HCA or Nursing Associate).
Provide weight management support and counselling, guiding patients through appropriate service tiers.
Identify and manage patients with non-diabetic hyperglycemia and pre-diabetes, offering lifestyle guidance to prevent progression.
Conduct NHS Health Checks, assess cardiovascular risk factors, and provide tailored health interventions.
Perform health assessments for new patients, recording their medical history and current health status.
Conduct mental health assessments and refer patients to the appropriate services as needed.
Perform health reviews for patients with learning disabilities, working with multidisciplinary teams (MDTs) to create tailored care plans.
Conduct reviews for long-term conditions, ensuring proper monitoring while maintaining existing medication plans.

Conduct urinalysis tests, interpret the results, and provide necessary follow-up care.
Fit and manage 24-hour blood pressure monitors; analyze the results and, if required, schedule a follow-up with the pharmacist for further review.
Administer medications and treatments under Patient Specific Direction, ensuring strict adherence to protocols.

Review and respond to queries related to nursing tasks from clinicians and non-clinical staff within 48-72 hours. Queries should be addressed to the nursing team.
Regularly check stock and supplies for expiration; order necessary inventory (including vaccines) and inform the Nursing Lead about faulty equipment. You can also raise a request to get something fixed using the Facilities Issues form on the Penrose Portal homepage.
Use this time for documentation, session preparation, and addressing inappropriate bookings or unresolved issues.
