What is this service?
The @home service provides hospital level care in a patients home. The service consists of health and social care workers that can provide medical, therapy or social care (depending on patient need). The service provides support for patients with heart failure, dehydration, COPD and unstable diabetes (full list of conditions).

Criteria
Inclusion
- Registered with a GP in Lambeth or Southwark or Lewisham
- Over 18 years old
Exclusion
The @home service is not suitable for patients that:
- are having an asthma attack
- need acute mental health care
- need suprapubic catheter care
- have injuries or lacerations that need suturing or gluing

Lambeth & Southwark:

Use this number for routine and emergency referrals (emergencies will be prioritised). Calls will be answered between 8am and 8pm, 7 days a week. Referrals are taken until 6pm. Only advice is given between 6pm and 8pm.

- Depending on the individual patients needs, the @home service aims to see patients within 2 hours of referral, the same day or the next day
