What is this service?
CCL is a free social prescribing service which supports patients to improve their health and wellbeing. The service helps patients find out about relevant local services and groups, and can provide support/information on more complex needs such as:
- transport
- mental health
- long-term health conditions (support is offered for patients and/or their carers)

Inclusion
- Resident of Lewisham
- 18 years old and over

Rejections, DNAs & other Quirks
- If you are referring a patient to this service, you must have their consent to do so
- The service aims to get in touch with referrers within 2 weeks, with the wait time for an appointment being 6 weeks. If support is needed more quickly, call the service directly.
