What is this service?
Social prescribing link workers connect patients to community-based services and groups which could help meet their practical, social and emotional needs. Link workers can help patients with issues such as:
- Healthy lifestyle advice and physical activity
- Low level mental health
- Social isolation and loneliness
- Employment, training and volunteering
- Low level addiction and recovery
- Managing a long-term health condition (e.g., diabetes, dementia)
- Getting benefits and financial support/advice
- Homelessness
Criteria
- 18 years old and over
- Registered with a Hills, Brooks and Dales PCN GP Practice
Referral may not be appropriate if the patients individual needs are too low or too high. Click the toggle below for further guidance on this. Assessing appropriateness of social prescribing

How to refer
🖥️ EMIS → Go to EMIS Tasks > ‘Add Task’ > Complete form > Send to Fiona McLees (Knights Hill Only!)
Rejections, DNAs & other Quirks
- Link workers can’t refer patients to weight management/reduction programmes. ForNHS Tier 2 Weight Management Service, The Lambeth Healthy Weight Hub, Up! Up! Lewisham (Tier 2 Weight Management) or NHS Tier 3 Healthy Weight Programme please refer directly to the provider.
- Link Worker appointment slots should appear on your practices’ EMIS appointment book. Do not add patient appointments here. This is only for use by the link worker.
