Long Term Conditions (LTCs) / Complex Medical Conditions usually involve multiple body systems and are chronic in nature. This means they persist over a long time, usually for a person's lifetime. There is usually no cure, and are managed with drugs and other treatments.
If a patient contacts the surgery asking for an annual review for any of the below Long Term Conditions (LTCs), this page will help you book an appointment with the appropriate person, as well as any other information the patient needs to know (e.g. the ‘Tests’ column are tests that will be carried out as part of the appointment - it may be helpful to let the patient know this in advance!)
