What is this service?
The Walking Away From Diabetes service, provided by Everyone Health Southwark, is designed to educate pre-diabetic patients on how to reduce their chances of getting type 2 diabetes.
The service involves attending a 3-hour workshop in-person, or two one and a half hour virtual workshops. The workshops are led by qualified practitioners who will explain exactly what type 2 diabetes is, and give patients advice on things like physical activity and healthy eating planning.
After completing the workshop, patients will be offered follow-up telephone evaluations to check their progress.
Criteria
- Aged 18 and over
- Identified as pre-diabetic
- Have an HbA1c result of between 6.0 and 6.4% (42-47mmol/mol)
- Registered at a GP practice within Southwark Borough

Rejections, DNAs & other Quirks
- The phone number for referrals is for patient self-referral only
- When making the referral to this service, give the patient their HbA1c/cholesterol and renal results so they can understand their values in relation to what is being taught
