What is this service?
The Lambeth Diabetes Intermediate Care Team (DICT), consisting of specialist healthcare professionals and administrative staff, supports Lambeth GP practices to help provide high quality diabetes care. DICT achieves this by providing several services, such as education groups and STEPS (type 2 diabetes prevention programme).
DICT also offers four specialist diabetes clinics across Lambeth: Gracefield Health and Social Care Centre, Springfield Medical Centre, Akerman Health Centre, and Northwood Group Practice at Crown Dale. Akerman Health Centre too provides a clinic to support patients with early renal disease in type 2 diabetes every Monday.

How to refer
🖥️ eRS → For a routine referral to any service provided by DICT, submit the form below to eRS (under 'diabetic medicine' → 'general diabetes management')

Rejections, DNAs & other Quirks
- DICT will respond to urgent referrals within 24-48 hours
- Patients should only be referred to STEPS after being offered the 'Healthier You' 9-month Diabetes Prevention Programme
- Type 1 or gestational diabetes should be referred directly to the hospital diabetes clinic
