What is this service?

Diabetes Book & Learn is a new service designed to improve access to diabetes education courses in south London. It provides face-to-face and online courses, including the HEAL-D course for African and Caribbean patients, as well as community support.

HEAL-D: is a group-based education and support programme for people with type 2 diabetes from African and Caribbean communities, developed to ensure content is culturally sensitive. The course focuses on advice about relevant cultural foods and other aspects of diabetes.

Criteria

• Adults
• Type 2 diabetes only
• All newly diagnosed patients must be referred into these programmes in the first instance.
• For HEAL-D: African or Caribbean heritage

Rejections, DNAs & other Quirks

• Type 1 diabetes courses will be available soon
• For appropriate referral, please state the following on the referral form in the ‘additional information’ section:
• patient’s ethnicity
• whether your patient would like to attend HEAL-D
• whether they are looking for more information or to book
