What is a care coordinator?
Our care coordinators work with a range of patients to help them navigate the health and social care system. To achieve this, they work very closely with a range of healthcare professionals, from clinicians at Penrose Health to care workers, therapists and doctors within a community or hospital setting.
A care coordinators duties include:
- Creating Personalised Care & Support Plan (PCSP) with patients to support them through their care journey
- Helping patients prepare for upcoming appointments with healthcare professionals, and/or supporting them after
- Advocating for patients during meetings with health and social care professionals
- Chairing Team Meetings (including both clinical and multidisciplinary meetings)
- Helping patients get access to the right services and resources at the right time, allowing them to take control of their own health
Booking a care coordinator appointment
Which patients could benefit from the support of a care coordinator?
Most often, care coordinators support patients:
- with long term conditions (e.g., , Severe Mental Illness (SMI), Dementia, Learning Disabilities)
- who require Palliative Care
- with impaired communication (blind/deaf/language barrier/speech problems)
- experiencing drug & alcohol problems, social isolation, homelessness, or who are housebound
When to book a care coordinator appointment for these patients
Patients with any of the conditions above can be booked with a care coordinator if they:
- receive input on their care from multiple teams and/or their care planning is uncoordinated
- are struggling to access social care, occupational therapy, their GP or hospital care
- have recently been discharged from a service or hospital and require a welfare check
- frequently attend multiple appointments with health and social care professionals
- need extra information about services
Care coordinators work closely with services such as social care, community physiotherapy, falls prevention services, community nursing, wheelchair service, community learning disability team, SLAM, memory service, social prescribers, health and wellbeing coaches and palliative care teams. Please note that the availability of services may differ by borough!
How to book an appointment
Care coordinator appointments should be booked via the EMIS appointment book. You do not need to send a task to the care coordinator at your site to book an appointment with them!
