What is this service?
LATT provides occupational therapy, physiotherapy and/or speech and language therapy rehabilitation for patients in their own homes (or community environment, where appropriate).

Criteria
Inclusion
- Aged 18 and over
- Live in Lewisham and/or have a Lewisham GP
- Have physical problems, speech and/or swallow problems, cognitive problems, and/or problems performing activities of everyday life
Exclusion
- A primary diagnosis of learning difficulties

Rejections, DNAs & other Quirks
- If a referral is marked as urgent, the patient will be seen within 48 hours. Wait times for non-urgent referrals can be anywhere between 2 and 12 weeks (dependent on need)
- This service does not offer maintenance therapy
