What is this service?
Lewisham NHS Trust District Nursing (DN) teams provide care in peopleβs homes and residential care facilities. They work closely with GPs, social services and other specialist teams, (e.g. tissue viability and continence nurses).
A team consists of a Neighbourhood Nurse lead (registered nurse with additional specialist DN training), Community Nursing Sisters, Community Staff Nurses, Primary Care Assistant Practitioners, Healthcare Assistants and an administrator.
Specialist clinics
The DN team accept referrals for a range of community clinics, which they process and pass to the specialist teams:
- Bladder, bowel and pelvic health
- Tissue viability
- Lymphoedema
- Leg ulcers
- Outpatient Parenteral Antimicrobial Therapy (OPAT)
Criteria
- Must be registered with Lewisham GP
- Adult (over 18 years old)

Rejections, DNAs & other Quirks
- Referral will not be rejected if βNeighbourhoodβ is not included on the referral form, however, if known you can include this information:
- KF β Neighbourhood 1
- For patients in Surrey Docks, consider where the patient lives; some patients will be in Lambeth, so contact District Nurse (Southwark & Lambeth)
- For blood test referral requests:
- TQuest forms must be sent as PDF attachments.
- they cannot accept TIF blood forms.
