What is this service?
The Community Neurorehabilitation Service, provided by GSTT, supports patients after a stroke or other neurological condition. The service is run by a multi-disciplinary team, which includes occupational therapists, speech and language therapists, nurses, and social workers.
Patients referred to the service will have an initial 45-minute appointment (via telephone, video or face-to-face depending on personal preference) to discuss their needs and goals. This allows the team to then provide a rehabilitation programme tailored to each individual. After 6 months, the team will review the patients needs, as these may have changed.
Criteria
Referrals are accepted for patients who:
- are aged 16 and over
- are registered with a GP in Lambeth or Southwark
- have a new stroke or other neurological condition which affects function
- have had a change in their neurological condition, such that it affects daily activities

Rejections, DNAs & other Quirks
- If you are concerned that your patient has a swallowing difficulty and is at risk of aspiration, please make sure this is clearly marked on the referral form. These referrals will be seen in 2-3 working days
- If an appointment needs to be cancelled or changed, advise the patient to call 020 3049 5512
