Outpatient services offered:

• Heart Failure with co-morbid frailty
• Dementia, cognitive impairment and memory problems
• Osteoporosis
• Fall Clinic
• Hypertension (via General Acute Medicine)
• Parkinson’s Disease
• Stroke Clinic

They also conduct Comprehensive Geriatric Assessments

Criteria

• Generally, >65
• For Heart Failure, and Hypertension: must be multi-morbidity
