How to initiate a referral
If you want to make a referral or advice and guidance request:
- write the body of the referral
- send a task to Clinical Admin with this and any other relevant information needed for the referral (e.g. recent blood test results, relevant history, clinic letters etc.)
For referrals to district nursing, task Reception team
If sending an urgent or 2WW referral:
- Please put a red flag or urgent on the task
- Open the clinical template on EMIS: 'Safety Netting Template (Updated 2022) PH', and fill in the 'Suspected cancer referrals' tab
- Create a document using the ‘2WW’ template, and complete
- Send the ‘2WW’ text on Accurx
