Managing Patient Personal Data
One of the most commonly received non-clinical requests is to update a patient's personal details/contact information. These changes can be necessary for various reasons:
- Moving house
- Name change by deed poll
- Marriage (change of patient name and possibly address)
- Adoption (change name of child and possibly address)
- Gender reassignment
Most of these changes can be done by the reception team, with the exception of adoption and gender reassignment, both of which must be done by the Reception Leads (see ‣ and Gender Reassignment on Patient Records for more info on these topics).
📄 Change of name requires physical proof
When a patient wishes to change their name, they must bring documented proof (marriage licence or deed poll) to the surgery to demonstrate the change. After witnessing this proof and recording it in the notes → change the patient data to match the document.
