Some medications are prescribed through a Shared Care Agreement (SCA). SCAs are made between a GP and a secondary care consultant (e.g., Psychiatrist, Cancer Specialist, etc.) when a patient’s condition is stable/predictable enough for the GP surgery to manage the ongoing treatment.

This is important to note for a few reasons:

Once the secondary team establishes the medication routine, it cannot be changed by anyone except the specialist.
These patients tend to require closer monitoring - many of the medication regimes require blood tests, etc. to keep track of the patient’s health.
Not all of these medications are oral - some are injections and may need a nurse to administer them.
SCAs can be found in patient notes on EMIS.

Blood thinning medication, also known as an anticoagulant, is a drug that slows down the body's process of making blood clots. It's prescribed to people who are at high risk of having a stroke or heart attack.
