- Any descriptive free text / document title → this should be written exactly the same as the Report type selection on Asana task, followed by which stage the request is at and date.
For SARs/brief summaries, once it is completed and sent, kindly add a consultation to the notes, including the code 'medical examination/reports' with a brief description under comments section about where it was sent (e.g. email address etc).
