Accessing the LCR
The LCR Portal can be found within the Portal Cerner (in the left column, Shared Records Systems section).
The Portal Cerner contains all the shared patient information from local hospitals, mental health services and community services, including upcoming appointments, and information such as allergies and medications.
The portal is an overview and the quickest way to find most of what you may be looking for (blood results, A&E/UCC, Discharges, etc.)
However, if you need to look further for SLAM, GSTT, King’s and Bromley Healthcare data, then use the LCR.
What’s in the LCR
The London Care Record contains documents, results and appointment history from hospitals, some community care and out-of-hours services.
- If you want to see documents/letters, results or appointments from a specific hospital, click on one of the tabs at the top right of the page (red circle).
- You will then see a list of the hospitals (purple box) that the patient has visited (or has an upcoming appointment in) and you can click on the hospital to view the information you require.
For example - if you want to see if a patient has a letter from a consultant they saw at GSTT, then click on Documents and then click on Guy’s & St Thomas’ Trust. All letters written by doctors at GSTT for this patient can be found in the drop down for this hospital.
Access to this data is useful because…
- Documents: if a patient says that they were told something by a doctor in the hospital and the GP surgery has not received this information via email, we can search the hospital documents here to locate the information.
- Sick Notes: if a patient says that they attended A&E and requires a sick note, you can see relevant hospital information here.
- Call Handling: if a patient rings the surgery confused about hospital appointments, you can find upcoming appointments here.
- Hospital discharge information can be found here.
- Radiology & Blood results: copies can be given to patients, but only if the results are negative or, in the case of non-negative results, they have discussed results with the doctor first.
- Safeguarding: you can find Social Workers/Mental Health care coordinators here to contact in mental health/safeguarding situations (e.g. if there is concern about a patient with history of mental health issues missing appointments).
- Alerting Clinicians: if a patient makes you aware of any hospital information that would be relevant for an appointment, after booking the appointment a comment can be made in the patient notes, requesting that the clinician “check LCR for further information”.
