Summary: A guide for AHPs and GPs on how fit note requests will be triaged by Reception to be completed by clinicians

How you’ll receive requests
Sent by task
When a patient requests a fit note via Reception, the request will be passed to the Reception Lead to triage and allocated to an appropriate clinician (following the procedure on our Processing Fit Note Requests page).
- Triaged requests will then be sent as tasks to the ‘Fit notes team’ for AHPs, or to individual GPs (if there are no relevant AHPs on site)
- AHPs check ‘Fit notes team’ tasks and issue fit notes that are relevant to them (e.g. Mental Health Nurse should issue fit notes for mental health conditions)
During an appointment
If a patient requests a fit note during an appointment with an AHP or GP, the clinician can issue a fit note immediately during the appointment.
How to issue a fit note in EMIS Web
- Open patient’s notes & add a new consultation
- Click ‘Add Fit Note’ → ‘New Issue - Not Fit for Work’ or ‘New Issue - May be Fit for Work’
- Complete the relevant information for the fit note
- When complete click ‘Save to Record’
- Send to the patient using Accurx
- Click the message icon in the Accurx toolbar
- Make sure that the correct patient has opened!
- Attach the fit note you just created from EMIS Web
- Check that ‘Save to record’ is ticked, then click ‘Send now’ to send to the patient
How to escalate an issue
If you are an AHP and have a question about issuing a fit note, use the following route to escalate queries:
- Ask your colleagues in the Fit notes team if they can help
- If they can’t help → ask the paramedic
- If they can’t help → ask the duty doctor
