Who’s it for: Care Coordinators
During a consultation, a patient may ask you to assist with housing issues (e.g., applying for council housing, homelessness, eviction). These queries can be handled by signposting to appropriate services and/or referral to social prescribing.
Signposting
You can direct patients towards appropriate housing support, by:
- Sending the ‘Housing support resources’ Accurx text
- Providing details of other relevant services, as below
- Advising them to speak with their local MP

Social Prescribing
Social prescribers can help patients with housing, benefits and financial advice. You can find details of how to refer to this service in our Social Prescribing page.
For Southwark, referrals to Social Prescribing should only be made if a patient is homeless, or at risk of homelessness.
Street Homeless Patients
If a patient is street homeless, offer support by:

- Contacting StreetLink
