What is GP2GP?
When a patient moves practices, they expect the GP to have their medical records for their first consultation.
- GP2GP allows records to be transferred immediately after registration, compared to the weeks it historically took to transfer paper medical records.
- This allows continuity of care between providers, and patients won’t need to recount a detailed history of their health.
- This also ensures patient’s current repeat and past medications are ready for the initial medication review.
Receiving: Three stage process
- Reception Team: Register the patient, in-line with Registrations, and perform a Personal Demographics Service (PDS) search, to find the patient’s spine entry. A successful search and match will make the request for their records. All patients must be searched on the PDS before registering without a match.
- Sending from the other practice is automated, and usually takes 2 minutes.
- Reception Team: Once received, the record needs to be integrated into EMIS.
- This can be found in Workflow Manager → GP2GP → Records Received
- Each record should be reviewed, and then filed (this must happen within 7 days)
- This process will inform the sending practice if they need to print copies of the record, before forwarding the Lloyd George envelope. They will automatically receive the labels for the envelopes. (See Patients Leaving & Deductions)
- If the sending practice is digitised, the envelope may be empty
Clinical Admin: Process ‘degrades’, check the records, and summarise the Lloyd George once received (See ‣).
- Degrades are data entries that cannot be translated, and this usually happens when the sending practice uses SystmOne or Vision. Should this happen, Clinical Admin will be tasked, and the paper records must be summarised ASAP. This should be prioritised over other records.
Pharmacists: As with all new patient registrations, check allergies & adverse reactions, and process regular prescriptions.
NB. Currently, this process is only used between practices which are in England or Wales. Paper records will be sent for patients from Scotland or Northern Ireland. These are traced by PCSE and delivered by CitySprint.
If we don’t receive notes through GP2GP
- Check for failed integrations or other errors
- Ring patient’s old practice and ask for a copy of the records to be sent via generic email - This should be attached to the patient’s records ASAP by Clinical Admin
- Clinical Admin must be tasked to summarize and transfer codes to the new EMIS record
- Paper records should be summarised as a priority, once received
What happens if a record is not integrated within 8 days?
The previous practice will be informed that the record has not been integrated, and a paper copy of the record should be printed alongside the Lloyd George.
This increases the quantity of paper records which needs to be processed and stored, and also causes avoidable work and expense.
