Summary of Role
The role of a Penrose Health Care Coordinator is to work with our clinical and non-clinical team to deliver the best healthcare service to our patients. More specifically, care coordinators will be responsible for helping the clinical team to provide more effective, efficient, personalised and joined up care to complex patients.

Additional responsibilities of a care coordinator lead
- Assisting in day-to-day management & supervision of Care Coordinators and their associated tasks
- The effective execution of practice systems and processes for high quality care
- Work with the central management team and HR managers in recruitment & performance appraisals

Personal attributes
- Strong organisational skills and effective time management – able to prioritise work and ensure that nothing is missed
- Strong verbal and written communication skills, necessary for interactions with a variety of agencies and patients
- Good notetaking skill
- Meticulous nature, maintaining a high level of attention to detail across sometimes repetitive tasks
- Analytical skills
- Fast learner who can follow processes and procedures accurately and efficiently
Knowledge, experience and qualifications
- NVQ level 2 or above
- Good knowledge of medical terminology
- Good understanding of the local health and social care system
- Strong keyboard and computer skills including Word and Outlook.
- Ideally would have experience working with the EMIS clinical system and E-Referrals System (ERS)
- Trained and experienced in minute taking
- Minimum of 1 year experience of working in multidisciplinary healthcare setting and patients’ interactions
